Description of Seminar
This course was developed in response to a need voiced by practicing Social Workers and Social Work Designees in long term care facilities. The course is designed to cover the function of social services from “A to Z”: to identify the role, function, and value of social work in LTC facilities; to provide material that can be the basis for social work practice in LTC facilities; and to comprehensively cover the psychosocial needs of residents and how to meet them.
Since a recent Department of Justice report indicated that residents’ social service needs are not being addressed in long term care facilities, survey agencies have been instructed by CMS to focus on this area during surveys. If you are new to the long term care field and need guidance, or if you are an experienced Social Worker in need of a “refresher” course, this is the training to attend!

Presenter Catherine R. “Cat” Selman, BS, ACC, - Cat Selman a consultant in her business, The Healthcare Communicators, Inc., is a well trained professional in the health care field with over 25 years experience. Ms. Selman received her BS Degree from Trevecca College and has completed graduate work at the University of Southern Mississippi. She served as Corporate Activity and Social Work Consultant for 101 nursing facilities in seven states before starting her own business in 1988. Cat has attended numerous CMS (HCFA) surveyor training sessions and has held positions on a variety of CMS (HCFA) workgroups: i.e. development of the MDS/PAC short-stay forum, abuse & neglect forum, revision of the survey, and interpretative guidelines and QOL initiatives.

Objectives •Identify components of the survey process.
•Develop appropriate policies and procedures for the social service department.
•Obtain required resident information by utilizing appropriate interview techniques.
•Complete appropriate sections of the Minimum Data Set (MDS) Assessment, utilizing guidelines, triggers & RAPs.

Session Objectives
•Identify components of a social assessment/history, social care plans, and progress notes.
•Formulate an appropriate plan of correction for case studies/social service deficiencies.
•Identify social service responsibility in regard to Advance Directives and Palliative Care.
•Develop a facility process to address needs of families and responsible parties.
•Formulate an individualized discharge plan for residents.
•Develop facility-specific in-service programs relating to social services.

Cancellation Policy Cancellations must be made in writing and may be faxed or mailed to KAHCF. Cancellations by phone will not be accepted. Cancellations received no later than May 17th will receive a full refund. Cancellations received between May 18th and May 24th will receive a refund of 1/2. Registrants who do not attend or who do not complete the full seminar will not receive a refund unless prior written notice is given to the KAHCF office.
