AVOIDING UNPLANNED HOSPITALIZATION

This webinar is developed to provide understanding of the processes, resources, and tools to avoid long term rehospitalization and methods of educating/training the staff who provide direct resident care on the process of managing the resident's acute condition changes to eliminate avoidable transfers to acute care settings.

Registration & Fees
$55 per Facility for KAHCF members
$75 per Facility for Non KAHCF members

No individual registration required, but we must have the contact person's name and email address. You may have as many people join in as you would like. One phone/computer may log in per registration.

When registering the Facility online, select only “0” to register your Facility. No individual registration is needed. A primary contact and email address are required.  We must have the email address of the person who will log-in. Registration will not be accepted after 12:00 p.m. on the day before the Webinar.

You'll need a phone and a computer with access to the Internet. Join the meeting using the instructions in the handout (or the email we send after registration).

Webinar Access Information
Once you have registered and paid for the webinar, you will receive two emails:
1. KAHCF confirmation email - immediately upon successful submission of your online registration.
2. GoTOWebinar.com email with log-in/call-in instructions - approximately one week prior to the scheduled webinar.
 
If you do not receive these emails one week prior to the scheduled webinar, call KAHCF for the access information and to ensure your registration was received. Registration will not be accepted after 12:00 p.m. on the day before the Webinar.

KAHCF Cancellation Policy:
Cancellations must be made in writing and may be faxed or mailed to KAHCF. Cancellations by phone will not be accepted. Cancellations received no later than January 26th will receive a full refund. Cancellations received between January 27th and February 2nd will receive a refund of ½. Registrants who do not participate will not receive a refund unless written notice is given to the KAHCF office.
